GTM Organizational Member Form

In 2023, GTM established a new Organizational Membership, allowing synagogues and other groups to enroll their volunteer musicians--singers and instrumentalists--as Associate Members, with access to many of the benefits of full membership, including attendance at Guild Talks Music webinars, access to GTM's private Facebook page, and member discounts at Transcontinental Music.  A full description can be found in this video:  Introducing GTM's New Organizational Membership.

The Associate Membership is for volunteer musicians.  Professional musicians should be Full Regular Members of the Guild with all the benefits of membership, including access to the ACC Retirement program.  They can also be enrolled on this form.  See the Professional Musicians section below.

ORGANIZATION INFORMATION


PRIMARY CONTACT INFORMATION
Enter here the name and contact information for the Primary Contact, typically the Cantor or Music Director.  This person must be a full member of the Guild of Temple Musicians at at cost of $50 for ACC or CA dual members, or $85 for Regular members.  See the Financial Information section below.
Enter here the name and congregational role (e.g. Cantor, Music Director) of the primary contact for the Associate Musicians.  This person will have the responsibility of communicating with the Associates.
Enter here the phone number for the primary contact.  It can be a business phone at the synagogue or a mobile phone.
Enter here the email address for the Primary Contact.  This is the address that will be used to log onto the GTM ShulCloud account, templemusicians.shulcloud.com. 


PROFESSIONAL MUSICIAN INFORMATION
Enter below the name, congregational position (e.g. Choir Director, Accompanist) and email address of each professional full-time or part-time paid musician whom you wish to enroll as a Full Member of the Guild.  Please put each musician on a separate line.  As soon as we receive this form, we will contact them to fill out a Member Update Form with all of their information.


ASSOCIATE MEMBERS INFORMATION
In the box below, please upload a spreadsheet or word document with the name and the email address for each Associate musician. 

Use the format FIRST NAME, LAST NAME, EMAIL ADDRESS.


FINANCIAL INFORMATION
Dues for your volunteer musicians are $360 annually for all your volunteer musicians no matter how many.  Dues for your professional musicians are $85 per member.   The Primary Contact must also be an individual member of GTM at $85 for Regular members or $50 for ACC or CA dual members. 

We also welcome you to join at the sustaining Chai level of $180.  Thank you for your support.

PASSWORD
When you click on Submit, you will be asked to create a Password for this ShulCloud account.  Please do not share this with any of your Associate musicians.  It provides access to all actions involving the account, including changing passwords and making financial transactions.  Only you and appropriate staff from your congregation should have access.

Please note that this password is not the same as the one used to access the GTM website, templemusicians.org.  You and each of your Associates will be issued a personal GTM website log-in, allowing access to Guild Talks Music webinars and other Guild activities.

You may either pay by credit card or you can bill to your new account, in which case we will send you an invoice.  You have the option of paying in up to four installments.  Final payment must be made by the end of our fiscal year, July 31, 2024.
